Your care, your voice
Patient rights and responsibilities
We believe care works best as a partnership built on respect, communication, and shared decisions.
Effective date: October 16, 2026
Riverbend Medical Group wants you to be an active partner in your care. We will listen to your questions, provide information in a respectful way, and work with you to make decisions. These rights and responsibilities apply to your interactions with our practice, subject to applicable law and the needs of safe care.
Your rights
You have the right to:
- Be treated with respect and dignity and receive care without discrimination.
- Have your privacy respected and your personal and medical information kept confidential as required by law and described in our Notice of Privacy Practices.
- Receive clear information about your diagnosis, treatment options, and expected costs in terms you can understand. You may ask questions if an explanation is unclear.
- Take part in decisions about your care and discuss the benefits, risks, and alternatives of recommended services with your care team.
- Give or refuse informed consent for care, and refuse treatment or withdraw consent to the extent permitted by law. If you refuse, we will explain the likely risks and consequences and discuss reasonable alternatives.
- Request a second opinion or ask for information about another qualified professional when you want an additional perspective.
- Receive a qualified interpreter at no cost when needed to communicate effectively, including an American Sign Language interpreter when appropriate.
- Know the names and roles of the people involved in your care and ask who is responsible for your treatment.
- Access and request copies of your medical records, and ask us to correct information as allowed by law.
- Request a Good Faith Estimate if you are uninsured or choose not to use insurance for a service. Learn more on our Good Faith Estimate and No Surprises Act page.
- Voice a question, concern, or complaint without it negatively affecting your care or the way our staff treats you.
Your responsibilities
To help us provide safe, effective, and coordinated care, we ask you to:
- Provide an accurate health history and tell us about your current medications, supplements, allergies, and changes in your health.
- Follow the care plan that you and your clinician agree on, or tell us if you cannot follow it or need help finding another approach.
- Arrive on time for appointments. If you need to cancel or reschedule, give us at least 24 hours' notice whenever possible so we can offer the time to another patient.
- Bring your insurance card and photo ID to visits, keep your contact and coverage information current, and pay applicable copays at the visit.
- Treat physicians, staff, other patients, and visitors with courtesy and respect.
- Ask questions when something is not clear and tell your care team if you do not understand instructions or have a concern about your care.
- Use appointment requests and other communication channels appropriately. Do not send sensitive medical details through the public website form or use a non-urgent channel for emergencies.
These responsibilities help us understand your needs and plan your care, but they do not change your right to respectful treatment or the privacy protections that apply to you.
